Prescription Renewal Policy

Most medications are renewed during your office visit for a 90 day supply without any additional refills. In the event that your medication was not renewed during your office visit, you may call for your renewals. Prescription drugs may be refilled by calling the officeat least 48 hours prior to running out of the medications. Please plan ahead as no prescriptions will be refilled on the same day. No “walk-in” prescription renewals will be approved while you wait. Prescription renewal is NOT an emergency.

Please be advised that most medications taken for extended periods of time require monitoring to evaluate your response and check for potential side effects. Therefore, the doctor may recommend that you make an appointment prior to any additional refills. As a general guideline, no medication refills will be approved if you have not been evaluated in the past three months.

Finally, no antibiotics, weight loss medications, and controlled substance prescriptions will be prescribed nor refilled over the phone. Patients must be evaluated in person for antibiotics, weight loss treatment, and controlled prescriptions.

 

Laboratory / Bloodwork / Test Results Policy

It is the policy of this office that bloodwork, laboratory results, and results of diagnostic testing are not discussed over the telephone under any circumstances. Please do not call the office to discuss your results with the doctor. Additionally, the staff is not medically trained nor authorized to discuss your results over the phone. You will be contacted by mail within 14 days with the details of the abnormality and any further recommendations. You will be contacted by telephone ONLY if your results are “critical” and require immediate intervention. You are encouraged to make a follow up appointment with the doctor should you wish to review any test results.

 

Missed Appointment Charge

Please be advised that we reserve the right to charge a fee of $25 (Twenty Five Dollars) for each missed appointment due and payable prior to your next office visit. An appointment is considered “missed” if it is not cancelled at least 24 hours in advance. In the event the office is closed the day before, the appointment should be cancelled the morning of that business day prior to 10 AM. It is calculated that a missed appointment with a physician costs approximately $45. The fee is an attempt to minimize “no-shows” and to partially offset ongoing office expenses, including staffing costs. Missed appointments result in wasting the doctor’s and staff’s time. The missed appointment time could have been used for someone else who needed it.

 

Insurance Plans & Payment Policies

We accept most health insurance plans including Blue Cross Blue Shield, Empire Plan, United Healthcare, Aetna, Oxford, GHI, and Local 1199. We do not accept any form of Medicaid. We will file your insurance claim for you at no additional cost but require that you provide us with a copy of your current insurance card. We also require that copayments and/or deductibles be paid at the time of service. We accept cash and checks. Personal checks are only accepted from established patients. We do not accept credit cards at this time and apologize for any inconvenience this may cause.

If copayments and/or deductibles are not satisfied at the time of your of visit, a $10 dollar administrative charge will be added to the amount of your copay to offset the time incurred for the additional staff time required for the extra paperwork and billing procedures.

 

Cancellation Timeframe

Please call at least 24 hours prior if you are unable to keep your scheduled appointment. This courtesy will allow us to utilize this appointment time for someone else. Please provide your name, date of birth, physician name, and appointment time to ensure accurate rescheduling.

 

Workers Comp & No Fault Insurance

We do not accept Workers Comp & No Fault Insurance.

 

Forms & Letters

We charge $75.00 for any paperwork, forms or typed letters up to 3 pages, that need to be filled out by the doctor.
